Maryland Master Gardener Annual Training Day May 28, 2009 Registration & Order Form

It is preferable that you register ON-LINE This will save paper, time and resources.

Registration and orders must be received by 5/1/09.
Please include fee of $59.00

For anyone registering after 5/1/09 the fee will be $69.00

Space is limited. Registrations will be accepted on a first come, first served basis.
Parking, morning refreshments and box lunch with a drink are included with registration fee.
**No Refunds after May 17, 2009**

To Register for Classes only, fill in the form above, write a check payable to the University of MD for $59 and mail to MG ATD, 12005 Homewood Road, Ellicott City, MD 21042. (Or you can use the form at the bottom of this document if you'd like to pay using a credit card.) Credit card registrations can also be faxed to Robin at 410-531-5567.

To Register for Classes and buy MG Identity Items, please register online.

If you need to pay by check, however you can use the .pdf form or the html form below. These forms must be printed out and mailed in with check or credit card information.
